    The key provisions are CPR 19.2, CPR 19.4 and CPR 19.6, and PD19A para 1.3 (in particular this states that an application to the court to be joined as a party must be supported by evidence setting out the proposed new party's interest in or connection with the claim).

    Evidence can be included as part of this form (at para 10), or attached as a separate witness statement (which must include a signed statement of truth, namely "I believe that the contents of this statement are true to the best of my knowledge and belief.")
