Needs assessment by social services


Adults social care services offer needs assessments to eligible adults who are over 18 and are City residents.

An assessment can point you to the right information and advice to assist you, or help you to access services that can enable you to be more independent and support you to live at home.

People who are eligible for assessment may have physical or mental health problems,a learning difficulty, problems with drugs and alcohol or may be an unpaid carer for a person with these needs.

If you would like an assessment you should speak to a social worker who can be contacted on 020 7332 1224. They may be able to help you on the phone or may organise a visit to discuss things further. The social worker will ask you for details about yourself to help find out more about your needs. Information given will be treated in confidence and shared with your permission.
