Adapting your home
Occupational therapy assessment
An important aspect of any care programme is an occupational therapy assessment.
The goal of occupational therapy is to improve your ability to complete everyday tasks and help you keep your independence.
Our specialist occupational therapy service works with you to assess and provide support you might need in the following areas:
- Personal care
- Personal safety
- Personal mobility
- Sensory needs
Occupational therapy can assist adults in long-term care and those recovering from a recent injury or illness.
A common outcome of an assessment is to provide you with equipment that can help you to carry out everyday tasks - that you might otherwise struggle with - and to maintain your independence. This could range from a frame to help you with walking indoors to assistive technology, such as a personal alarm to detect if you need help at home.
Help to remain at home
The Adult Social Care Team will carry out an assessment with you to determine:
- your eligibility for care
- what level of services or equipment you require to enjoy the best possible quality of life
- if you will have to pay for any services via a financial assessment.
The social worker may refer you to the Reablement team. This team can provide up to six weeks of short term home care support which is not charged for (regardless of any savings/property you might have).
If you are having problems managing at home or are coming out of hospital, you can contact the Adult Social Care Team on 020 7332 1224 about your options.
