Whether you have just left hospital or are recovering from a recent illness or injury, our Reablement service provides short-term, intensive support to help you with everyday tasks. It also works to restore the skills, confidence and independence you may have lost.

It’s delivered in your home and focuses on helping you to achieve everyday tasks so your independence, health and wellbeing aren't affected in the long term.

You may be able to use the Reablement Service if you live in the City of London, are aged over 18 and feel you would benefit from services that will help you to gain or regain your independence.

You may be referred to us by the hospital you are in, your GP or another professional.

Alternatively, you can contact us for an assessment if you feel that some help will boost your confidence and ability to get on with everyday tasks. There are a range of care and support services on offer. We can provide:

  • Equipment to assist you with getting around and other everyday tasks
  • Advice on how to best manage your personal care
  • Support to help you regain your confidence and independence

The Reablement Team will discuss your needs with you to find out what you feel able to do and what may be causing you some difficulty. Together, we will complete an assessment of your needs and draw up a plan outlining the care, support and therapy you need to improve your current abilities at home.

Your plan will be based on your individual circumstances. It will be tailored to meet your needs, focused on helping you to remain in your own home and live as independently as possible.

The City Corporation's Adult Social Care Team provides the Reablement service which includes an Occupational Therapist and Reablement Coordinators.

We provide the service in partnership with health services and, where appropriate, with community health professionals, such as district nurses and physiotherapists.

We provide the Reablement Service for up to six weeks. A member of our Reablement Team will regularly look at the service that you receive to ensure it meets your needs and helps you to do all the things you need to do in your everyday life.

Reablement services are free of charge for up to six weeks. If your needs continue after this time, the Reablement Team can discuss further steps with you.

Once you are able to achieve those everyday living tasks and feel that you have regained your independence, the Reablement Team can refer you to local services that can offer you further support.

If your needs are greater and you need some ongoing care and support, the Reablement Team can put you in touch with a social worker who can advise on the best way forward.
