Areas covered by re-ablement

There are a range of care and support services on offer. We can provide:

  • Equipment to assist you with mobility and other everyday tasks.
  • Advice on how to best manager your personal care.
  • Support to help you regain your confidence and independence.

The Re-ablement Team will discuss your needs with you to find out what you feel able to do and what may be causing you some difficulty. Together, we will complete an assessment of your needs and draw up a plan outlining the care, support and therapy you need to improve your current abilities at home.

Your plan will be based on your individual circumstances. It will be tailored to meet your needs, focused on helping you to remain in your own home and live as independently as possible.

If your needs are higher and you need some ongoing care and support, the Re-ablement Team can put you in touch with a social worker who can advise on the best way forward.
